"A special way of caring"

To support and complement existing services involved in patient care.
To help to provide equity of care and choice for patients who wish to remain at home.
The Team:
Trained nurses who have specialist knowledge and skills in order to provide palliative nursing care in the patients own home.
Auxiliary nurses who have experience in caring for palliative patients who are dying along with offering support to their families.
Volunteer sitters whilst not offering hands on care, will sit with a patient whilst the carer has a break or attends to other daily neccesary routine chores.
What do we offer?
Periods of care over twenty four hours a day, seven days per week.
Help with hygiene, medication, symptom management, support and advice for family, carers and health professionals.
Who can use the service?
Patients who have a palliative diagnosis that require support following a complex discharge or in the final stages of life who have palliative needs, i.e. there is no known cure and are no longer seeking active treatment.
Referral to the service.
Referrals are accepted from the patients G.P. or district nurse.
Every G.P./district nurse base has a referral form specifically designed for Hospice at Home.
Completed forms should be faxed to the Hospice at Home team.
Referrals will be dealt with between 10am-4pm daily. Referrals after this time will be for the next working day.
Hospice at Home will liase with the G.P./district nurse and allocated the appropriate available care.
